I have already called the phone company and had them run a line test. They called back and said the line is clear. I have communicated the problem with my ISP and say the problem is not theirs, but with the phone line. The cable from the phone jack to my PC is a "Monster" brand cable made specifically to help reduce line noise and give the best connection. It runs straight back from the phone jack to the PC, so there is no place for it to be abused in any way.
Regarding a filter, I don't have one. I maintain a separate line for the house phone.
Any more suggestions? I'm almost willing to try about anything at this point.
