Modem problem

i have problem with my new modem.

HSP56 MicroModem
Driver Provider: PCtel
Driver Date: 1/15/2003
Driver version: 11.300.6.0

the modem installed properly and i have installed the driver. but when i try to connect to the internet (dial-up), windows gave me this error code:

then i try run Query Modem but i got this:

how can i solve this problem?
