'A' drive problem - any ideas appreciated

1) When formatting new floppy Disks get error ‘windows unable to complete the format’ and ‘disk in drive ‘A’ cannot be formatted’ get same error when trying the quick format option.
2) When trying to copy to an unformatted new floppy; get error ‘the floppy disk controller reports an error that is not recognised by the floppy disk driver’
3) When opening previously copied floppies they can be accessed without problem.
4) When trying to add files to a previously copied floppy (with free space) get error ‘this file could not be found’ etc.etc

PC Running XP SP2 Pro. All updates/patches current.
Floppy driver version 5.1.2.2600.2180 (XPSP_SP2_RTM 040803 – 2158)

I would try another floppy drive. These things are real cheap.
