I would recommend Using PHP, and Mysql.
PHP is a programming language for just such needs. it has good integration with Mysql Databases. Providing many functions for connections with databases. To make things even easier, You could try a combination of PHP, MYSQL, and Macrmoedia Dreamweaver. Macromedia has built in scripts for things like that. It will even create an input form automatically from any table within a database. I think this is your best bet.
EDIT: I forgot to mention that the server where the web page with the input form resides, has to support PHP, asnd MYSLQ, most web servers these days do. If you are using a web host service, then you shoudl check with your procider to make sure they are supported.
