PDF Forms

Want to create a PDF form that I can email to other users for them to fill in the "blanks" and then mail back to me INTACT. DO NOT, NOT NOT need XML file or data to be pulled into a database. Do not have or want this linked to any database--just want to have a form that another person can fill in and mail back with nothing changed on the form except the fields where input is filled in.
