Closing a program with C-A-D

I’d appreciate it if someone would explain what goes on when I use the Ctrl-Alt-Del keys on my W98 pc to close a program. In particular:

 When I click CAD I get a screen titled “close program” that lists my open programs. When I highlight the one I want to close (say APPLE) and then click the END button, most of the time I get a second screen, titled APPLE, with its own End button. And, most of the time, when I click that END button the APPLE program gets closed.

 But not always. Frequently I have to do a CAD and End several times before the first screen disappears and the second screen appears.

 And sometimes the program will close without my ever seeing the second screen.

I’m curious as to why the seeming inconsistency.
